In compliance with government regulatory agencies, we maintain a complete file on all of our valued carriers. In order for us to complete a file on your company, you will need to complete and provide the following information:

1. Signed “Carrier and Broker Agreement”. Please review, sign, and fax or email back the contract. Our email address is: dispatch@sockeyefreight.com.
2. Certificate of Cargo Insurance naming Sockeye Freight as the certificate holder (you may send us a generic form prior to getting us listed as a certificate holder).
3. A copy of your Motor Carrier Authority.
4. Your Taxpayer Identification Number (W-9). If you need a blank W-9 form, there is one below (it is also included in the password protected carrier agreement).
5. A list of your references and/or company information (if available).
